$50 Rebate
This rebate is available to first-time campers, returning campers who bring a first-time camper with them, and children of cottagers surrounding Lake Louise. Mail in this rebate form to receive $25/$50 off your camp registration.

Camper Registration
The registration form must be filled out completely and accompanied with the minimum deposit of $75 for each camper in order to register. Registrations are accepted until camp is full. Deposit is non-refundable and non-transferrable. Registration must be paid in full at the check-in at the beginning of camp.

Camper Release
This form must be completed, signed and returned before your camp session begins or bring to camp. The State of Michigan requires that we have a plan to assure that campers leave camp only with authorized persons. If camper is driving to and from camp and under the age of 18, parent/guardian must still complete this form.

Health History
The information in this form is to assist your camp staff in identifying appropriate care. Parent/guardian should fill out pages 1-3. Any changes to this form should be provided to the camp health officer upon check-in at camp. This form may be photocopied for your convenience.

Horseback Riding Release
This form must be completed, signed and returned in order for the camper to participate in any horse back riding activities.

Scholarship Form
Lake Louise's Scholarship Fund is supported by private donors and is intended to help those who might be financially excluded from attending camp. Our goal is to help as many youth experience camp as funds will allow. Scholarships are limited to $100 per camper. Funds are limited and scholarships are granted on a first come, first served basis. To be eligible for scholarship consideration the camper must be registered for camp and the deposit must be paid. All information is kept confidential.
